In order to make your first visit as smooth as possible, please print and fill out all New Patient paperwork and bring it to your first appointment. If you are unable to fill out the forms ahead of time, you may need to arrive 15 minutes early to complete the forms on site.

The First Visit
You’ll meet with Dr. Derik for a consultation, during which, he will discuss your health history and what has brought you in. A thorough examination should reveal the cause of your problems. You will receive a full report based on Dr. Derik’s findings and he will discuss your personalized treatment plan. This may include in-home exercises, dietary changes, and nutritional supplement support in addition to chiropractic adjustments.
Dr. Derik typically gives an adjustment on this visit if X-rays or other in-depth testing are not needed, so please wear comfortable clothes that allow full movement. Tight jeans and short skirts should be avoided.
Dr. Derik enjoys getting to know you and designing a plan specific to your needs, so please allow 45 minutes to an hour for your first visit.
The Second Visit
At your second visit, Dr. Derik will review what was discussed at the first visit and recheck any of the findings from the initial exam. He can answer any questions you have regarding your treatment plan and fine-tune it if needed.
Please allow 15-20 minutes for this visit.
Dr. Derik will adjust you and talk to you about your progress. Each follow-up appointment uses a variety of chiropractic and natural healing techniques that allow the doctor to customize to your needs and preferences specific to that day.
Re-Evaluation
In order to ensure that your treatment plan is meeting your continually changing needs, Dr. Derik will perform regular evaluations every 4-6 visits.
Re-evaluation appointments take approximately 30 minutes and are vital to make sure your body is moving in the right direction.

As long as you have stress in your life, your body will have to adapt and compensate for these pressures. Chiropractic care, unlike many other options available, helps to remove these stresses. Many patients realize how much better they feel when they are adjusted regularly and choose to do so.
